Change of Enrolment Form

This form should be considered as your official request to change courses, and does not imply your acceptance into the new course. Please make contact with the School Adminstrator for your new course and discuss with them your proposed change.
Changing the delivery mode of a subject can only be done prior to census. Please confirm the census date has not passed for this subject: *
A deferral from study is for a maximum of 2 semesters. At the end of your deferal period you will need to contact Student Services either to reactivate your enrolment or withdraw from your study.

You may be eligible to apply for a Compassionate Withdrawal on your transcript if your circumstances are both extenuating and beyond your control (such as severe illness or bereavement). You would need to supply independent evidence of this. Would you like to apply for a Compassionate Withdrawal? *
You will need to supply evidence to support your request. This can be brought in to Reception at 181 Goodwood Road Millswood or emailed to studentservices@adelaide.tabor.edu.au

I understand that I must return my student card to Student Services as I can no longer use it to claim student discounts.
