IMPACT LENDING APPLICATION INTAKE FORM

Thank you for your interest in a small business loan through the Impact Lending program. To begin the loan request process, please complete all five sections of the following Loan Application.
 
Upon submission of this form, someone from our team will reach out with a list necessary pre-screen documents you will need to submit. 
 
Your application will not be processed until all sections are complete and all required documents are submitted to Eric Uva (euva@liscsb.org). Incomplete applications will be automatically declined 30 days after application start date in compliance with the Equal Credit Opportunity Act (ECOA).

Additional documentation may be necessary subject to Lender's review of application.
