This program is designed for youth who struggle with social skills, life skills, anxiety, depression, and other mental health issues. Please tell us a little about your child and why they should be a grant recipient for this program. if the participant has been diagnosed by a medical professional with any cognitive, emotional, or physical disorders/ailments, please list them here. (Participants are NOT required to have a clinical diagnosis to be eligible for this program or the grant.) *

Is there anything else we should know about the participant? *

Scholarship funding is not always available. Availablity of funds is dependent upon current grant monies that have come to USHWR and some grants may be specific to certain programs. Decisions are made by the Scholarship Committee of the Board of Directors on a rolling basis.
