Basic Eligibility

1) Can you provide documentation supporting your status as an active duty service member, veteran, military spouse, caregiver, or surviving spouse? (Documentation can include a letter from your commander, your DD214, current military orders, or any comparable proof of service.) *
2) In the last 3 months, have you experienced one of the following that has/will impact you financially?

- A state/local natural disaster, such as house fire, flooding, tornado, etc.
- A personal financial crisis that you will explain in detail on this application. *
3) Are you able and willing to provide The MOAA Foundation proof of use of grant funds if awarded, within 30 days? *
4) This grant program does not currently provide assistance to businesses. Are you applying for this grant program strictly for business-related expenses? *
5) Do you attest that your discharge from uniformed service is either Honorable or General Discharge Under Honorable Conditions and you can provide official documentation verifying this discharge status?

(Please note: If you check yes and your application for funding is approved, you will be required to send documentation supporting this claim. If upon receipt, discharge status is anything other than Honorable or General Discharge Under Honorable Conditions, you will not receive a grant.) *
