Appointment Length: 55 minutes

Fee for RUNNING Initial Assessment: $199 per athlete

Fee for REGULAR Initial Assessment: $149 per athlete

All follow up trainings will be scheduled and billed separately.

Eligible Participants: All gender athletes age 11 and older

Thank you for your interest in Operation Zero's injury risk reduction programs!

Prior to your scheduled appointment, each athlete or parent/guardian (if athlete is under 18) must complete the following form in its entirety. The information you provide will only be used for program-related safety, communication, and data analysis.

To ensure adequate screening time, all forms must be completed and payment received one week prior to your scheduled appointment. Participation cannot be guaranteed for those who do not meet this deadline.

Please allow 10 minutes to complete all forms and provide payment information.

As a program participant of OPERATION ZERO, your screening and training will be administered by licensed physical therapists and/or physical therapist assistants. This is not a physical therapy evaluation or treatment. If you have a condition that is beyond the scope of this screening, we may suggest that you consult a physician or obtain a referral for a physical therapy evaluation.
